REGISTRATION FOR MIDLAND RESIDENTIAL STUDENTS
REGISTRATION FORMS ARE AVAILABLE FOR STUDENTS IN GRADES K4 - 12TH.  ALL MIDLAND RESIDENTIAL STUDENTS MUST BE REGISTERED IN THE DISTRICT INCLUDING ALL HIGH SCHOOL AND CHARTER SCHOOL STUDENTS

Children must be 4 years of age on or before September 1 to enroll in the Kindergarten 4 program and
5 years of age on or before September 1 to enroll in the Kindergarten 5 program

Documentation required for all students enrolling in the Midland Borough School District:
Proof of Student's Age (Birth Certificate or Passport)
Current Immunization Records
Prior Academic Records (for transfer students)
Proof of Residency (Acceptable documents to establish residency include a deed, lease, mortgage statement, current utility bill, or tax receipt with the Midland address)
and a current valid PA Driver's License or PA State ID with the Midland address
